Overview

After a Community Care Assessment has taken place and the requirements for Home Care Services have been identified, the request is passed to the Home Care Brokerage Team. The Brokerage Team will then make contact with the Social Services Home Care staff and the external providers of home care, with whom the Council has contracts, in order to provide the best care. The Home Care Teams may include women and men from all sections of the community.
