Southend-on-Sea Borough Council Collaborative Care Team is a domiciliary care agency, which provides short-term rehabilitative care to adults in their own homes. The care provided by the agency is focussed on the service users’ recovering or retaining their independence by being supported in personal care tasks, daily living skills and the re-building of confidence. The agency is part of the Integrated Care Service and receives referrals from the hospital and the community. There are currently 40 individuals accessing the service, but this changes on a daily basis owing to care being provided for a maximum of six weeks. The staff team is multi-disciplinary and includes the registered manager, a community matron, registered general nurses, therapists and health and social care workers. The offices are situated within the Cumberlage Intermediate Care Centre in Southend-on-Sea. This service is a free service.
